Try one of John's forked version firmwares (based on older Merlin). I found 13E1to be the most stable but haven't tried anything newer than 15.

I suspect that the firmware you tried had a CFE update inside and it somehow failed. Otherwise, if the CFE was intact, it should be recoverable. One of the great features of the Asus CFE system and it's ability to recover. All breaks down when the CFE is being updated and that update fails...
No, you're not stuck but you'll have to change your CFE from TMobile 2.1.2.6 to Asus 1.0.2.0 (or 1.0.2.1). If you have Telnet ability, you won't have to do anything except follow the guide to slide into an Asus CFE. If you don't have Telnet, you'll have to downgrade to an older TMobile firmware...

First two MAC's are the same. I would enter the one on the back into the first two. As for the other, you could take a guess (or pull one from an old router in the house). You could also just change the last two digits (as that seems to be the pattern anyway - off by a few digits).
I would suggest an NVRAM reset (press and hold the WPS button while powering router on and continue to hold WPS button for at least 10 seconds or so after powering on). Also, your IP address is static, correct? Finally, try a different browser.
