Modem home page 404

Not sure if this is the correct sub-forum but Today I went to access my Modem home page and I continue to get 404 Page Not Found. I rebooted the modem and my router, and still the same. I hit the reset button on the back of the modem and it continues to be an issue. I called my provider and they think it's on their end but I'm confused as to how that is. I thought I connect directly to the modem from the router.

EDIT: I still have internet access but I'm a little concerned that my modem might be compromised?
