That was added in 380.59.

Is there a way to change these configurations in ssh or telnet without going through the ui on 380.58? I've tried

grep -r "passthrough" /etc

to find a conf file and wasn't sure if that's going the wrong direction.

No, it was a firmware-level change. The code is simply not there in 380.58.

Should I disable the SIP passthough and redirect the ports manually ?

If you are still working on this you should see if your SIP client has an option to automatically detect its public ipv4 address or a field where you can enter it manually(you can actually point it to a asusdns entry).
You could forward the signalling port which is often 5060 and you should forward the rtp ports your client wants to use.

It is actually shocking how many users have their issue "fixed" by SIP Pass through/ALG doing layer 5 NAT on the SIP messaging. For those users it would be trivial to hijack any telephone call they make.
